Artist Forms and Registration
Congratulations on becoming part of the San Diego Delegation to the 2012 JCC Maccabi Artsfest.
Step 1.
Download the form below, fill it out, and fax it to 858.457.2422, Attn. Karen. Due January 15th

Step 2.
Once you are confirmed as part of the delegation, Register online as an Artist for the JCC Maccabi Artsfest 2012
Go to www.jccmacreg.org to register.
*If you’ve participated in the JCC Maccabi Games or Artsfest in the past, you already have log-in information. Do not create a new user. Contact Karen Flexer Friedenberg, Delegation Head, to get your log-in information.
Step 3.
Download the attached Artist Confirmation Kit. Fill out required forms in the kit and submit them to Karen Flexer Friedenberg in person, via email, or by mail to:

Artsfest Medical Examination Form (Due May 1st)
(NO OTHER FORM WILL BE ACCEPTED)
- Code of Conduct Signature Page
When submitting forms, please ALSO submit:
- Copy of Birth Certificate or Passport for age verification
- Copy of Health Insurance Card (cannot participate without insurance) & Prescription Card if separate.
DUE BY MARCH 15th
Late confirmation kits will result in a $125 fee.
