Step 1.
Download the form below, fill it out and fax it to 858-457-3866, Attn. Sarah Due by February 20th.
Step 2.
Register online as an Athlete for the JCC Maccabi Games.
Step 3.
Download the attached JCC Maccabi Games Confirmation Kit.
Fill out required forms in the kit and submit them to Sarah Zides in person, via email (sarahz@lfjcc.com) , or by mail to:

Forms in the Confirmation Kit that MUST be completed and returned are:
JCC Maccabi Games Medical Examination Form (NO OTHER FORM WILL BE ACCEPTED)

When submitting forms, please ALSO submit:
Copy of Birth Certificate or Passport for age verification
Copy of Health Insurance Card (cannot participate without insurance) & Prescription Card if separate.
