Download the form below, fill it out and fax it to 858.457.2422, Attn. Karen Due by January 15, 2012

Step 2.

*If you’ve participated in the JCC Maccabi Games or Artsfest in the past, you already have log-in information. Do not create a new user. Contact Karen Flexer Friedenberg, Delegation Head, to get your log-in information 858.362.1331, karenf@lfjcc.com

Step 3.

Download the attached JCC Maccabi Games Confirmation Kit. Fill out required forms in the kit and submit them to Karen Flexer Friedenberg in person, via email, or by mail to:
